The diaphragm is a dome-shaped soft rubber device with a flexible rim that covers the cervix and prevents sperm from entering the uterus and fertilizing an egg. It is inserted into the vagina before intercourse and is used with spermicide to kill sperm that get past the barrier. You must be fitted for a diaphragm at the health center and we will give you a prescription which you can fill at a pharmacy.
The diaphragm is inserted up to 6 hours before having sex. More spermicide must be used each time a woman has sex while wearing the diaphragm. Additional spermicide should also be used if a woman is going to have sex more than 3 hours after she inserted the diaphragm. After sex, the diaphragm must be left in for at least 6 hours, but no longer than 24 hours.
How to decide if the diaphragm is a good choice for you:
- This form of birth control is not as reliable as other forms and if you use it, you must have a good supply of spermicide.
- If you use the diaphragm, you must take responsibility for checking it regularly for holes or weak spots, and replacing it as needed.
- If you can take responsibility for sex in advance, then the diaphragm is a good choice.
- The diaphragm isn't good for anyone who is uncomfortable or uneasy with the thought of reaching into her vagina.
- The diaphragm may not be a good choice if you have certain medical conditions, such as frequent urinary tract infections (UTIs).

The diaphragm is 84-94% effective. You must use spermicide each time you have sex.
How do I get the diaphragm?
You must get “fitted” for a diaphragm by a health care provider. You need to be “refitted” with a weight loss or gain of more than 10 pounds. A diaphragm should be replaced every 2 years.
What are the advantages and disadvantages of the diaphragm?
Advantages:
- Can last several years
- Costs very little to use
- Can be used while breastfeeding
- It can be left in place for up to 24 hours
- It is non hormonal
- No effect on menstrual cycle
Disadvantages and side effects:
- Must be used every time you have sex
- Must be used with spermicide; you must use more spermicide for each time you have sex
- Using spermicide nonoxynol-9 may increase risk of HIV infection
- Cannot be used if you have an allergy to latex or spermicide
- Should not be used during vaginal bleeding or infection
- Increased risk of bladder infection
- Needs to be refitted with weight gain or loss of 10 pounds or more
What are the warning signs of problems with the diaphragm?
Warning signs include:
- Recurrent vaginal irritation, with no evidence of vaginal infection, may indicate an allergy or sensitivity to spermicide, latex, or silicone
- Discomfort with diaphragm in place
- Frequent bladder infections
- Unusual vaginal discharge
- Vulva/vaginal redness or swelling
SPECIAL NOTES
The diaphragm does not protect you against STDs (sexually transmitted diseases). Use condoms and spermicide along with the diaphragm to prevent transmission of HIV and other STDs. S Have a pregnancy test promptly if you suspect you could be pregnant.
